Important information about insurance for child care providers

Dear Child Care Provider:

As you know, on October 1, 2010 the Homecare Union Benefits Board (HUBB), in conjunction with Family Health
Insurance Assistance Program (FHIAP), began offering child care providers health insurance to SEIU Local 503
represented child care providers. The insurance contract with ODS and Kaiser Permanente required a certain level
of participation for the plan to remain active. Our level of enrollment did not meet the minimum participation
levels required for the plan to remain viable.

Unfortunately, due to the low volume of enrollment, the SEIU Local 503 Child Care Providers Plan has been
discontinued and we can no longer accept insurance applications for the program.

If you applied and you were not approved for coverage prior to April 1, 2011 there is no longer an option for health
insurance enrollment on the SEIU Local 503 Child Care Providers Plan.

If you were approved for coverage prior to April 1, 2011 we are working with the insurance carriers and FHIAP to
find a possible alternative plan for you. We will notify you when your SEIU Local 503 Child Care Providers Plan health
insurance coverage ends and what alternatives may be available to you.

You should also be aware that SEIU Local 503 continues to be committed to securing quality affordable healthcare
for child care providers. Due to current economic conditions and cost, healthcare is a challenging issue at the
bargaining table with the state. Currently the elected SEIU Child Care Bargaining Team is working on creative
options for healthcare coverage. If you want more information on bargaining or want to get involved please call
